Yes. We complement, not replace, your PCP. You’ll still see your PCP for acute issues, preventive screenings, and urgent care.
Insurance & Payments

No. Kultivate Women’s Health is a private-pay practice so that we can give you longer visits, personalized care, and access to advanced testing without an insurance company getting in the middle of you and your care.

Yes. You can pay with your HSA or FSA card at checkout.

Absolutely. After your visit, we can provide a detailed superbill with all the necessary billing codes so you can submit a claim to your insurance company. Please note that most insurance plans do not reimburse for menopause care or medical weight loss services, though some may allow you to apply these out-of-pocket expenses toward your deductible.

No. Our providers have opted out of Medicare and do not bill Medicare or Medicare Advantage plans for office visits. This means:
You pay out-of-pocket for visits at Kultivate Women’s Health.
Medicare and supplemental (Medigap) plans generally will not reimburse these visits.
If you have Medicare, we’ll guide you on which labs and services can still be covered elsewhere.
Labs & Testing

Yes.
We can route lab orders through your insurance. However, every insurance plan is different, and we cannot guarantee what your insurer will cover, apply to your deductible, or reimburse.
It is your responsibility to verify coverage, network status, and potential out-of-pocket costs with your insurance company before having labs drawn. Some patients are surprised to learn that extensive hormone, thyroid, metabolic, or specialty testing may not be covered by their insurance plan and can result in significant out-of-pocket expenses.
Kultivate Women's Health is not responsible for charges incurred by your laboratory or insurance company, including denied claims, deductible expenses, co-insurance, or non-covered services.
For this reason, we often recommend our cash-pay lab packages for patients with high-deductible health plans, limited laboratory coverage, or uncertainty regarding their benefits. Cash-pay pricing provides transparency and helps avoid unexpected bills. If you have met your deductible for the year or have excellent laboratory benefits, using insurance may be a reasonable option.
If you are unsure which option is best, please contact your insurance company directly or discuss your concerns with our team before completing your laboratory testing.

Yes. If you prefer upfront, discounted pricing, we also partner with a direct-to-consumer lab that offers clear self-pay rates and multiple convenient blood-draw locations. Many patients choose this option for cost transparency and speed. HSA/FSA cards can be used to purchase labs.

Absolutely. If you’d like to complete labs before your visit, we offer curated cash-pay lab panels through our office with transparent pricing.
Many patients choose our Comprehensive Baseline panel so we can review results together and make the most of your visit time.

Please note: these cash-pay lab panels are not billed through insurance. If you would like labs ordered through insurance, you’ll need to complete a visit with your provider first so appropriate orders and diagnoses can be discussed.
Hormone & Midlife Care

Yes. We specialize in bioidentical hormone replacement therapy (HRT)— estrogen, progesterone, and testosterone — when clinically appropriate.
We use FDA-approved or carefully compounded formulations for precise dosing and easy adjustments. We do not place hormone pellets. We can provide care to women who want to transition off hormone pellets.

Perimenopause, menopause, thyroid concerns, weight management, sexual health, vulvar conditions and overall midlife wellness.

Yes. Men are welcome for testosterone replacement therapy (TRT) and medical weight loss with our Family Nurse Practitioner, Holly DeWalt, FNP.
Holly specializes in men’s and women’s hormone optimization and weight management.Please note that Jean Marino, WHNP-BC, and Ann Konkoly, WHNP-BC, do not see male patients—but Holly is happy to see your husband or partner and provide the same evidence-based, personalized care we offer all patients.
Appointments

At Kultivate Women’s Health, we believe your time—and ours—is valuable. We’ve found that collecting payment when you book your appointment dramatically reduces no-shows and last-minute cancellations to less than 5%, which means we can offer more consistent availability and on-time scheduling for everyone.
Because we’re a private-pay practice, we don’t double-book or rush visits to make up for cancellations. This policy allows us to keep the schedule running smoothly, dedicate the full appointment time to you, and ensure that each patient who wants care can be seen promptly.
We appreciate your understanding—this simple step helps us keep our focus where it belongs: on you and your care.

Initial visits are 60 minutes; follow-ups are typically 30 minutes. We focus on education, shared decision-making, and giving you plenty of time to feel heard and supported.

Yes. We have membership options for ongoing support, priority scheduling, and special pricing on services.

Yes. Many appointments, including hormone consultations and follow-ups, can be done virtually or by phone, as needed. Procedures and physical exams are completed in-office.

Because we reserve significant time for each patient, we ask for 48 hours’ notice to cancel or reschedule. Late cancellations or no-shows may incur a fee. Please text the office at 216-877-8879 for cancellations.
