Yes. We complement, not replace, your PCP. You’ll still see your PCP for acute issues, preventive screenings, and urgent care.
Insurance & Payments

No. Kultivate Women’s Health is a private-pay practice so that we can give you longer visits, personalized care, and access to advanced testing without an insurance company getting in the middle of you and your care.

Yes. You can pay with your HSA or FSA card at checkout.

Absolutely. After your visit, we can provide a detailed superbill with all the necessary billing codes so you can submit a claim to your insurance company. Please note that most insurance plans do not reimburse for menopause care or medical weight loss services, though some may allow you to apply these out-of-pocket expenses toward your deductible.

No. Our providers have opted out of Medicare and do not bill Medicare or Medicare Advantage plans for office visits. This means:
You pay out-of-pocket for visits at Kultivate Women’s Health.
Medicare and supplemental (Medigap) plans generally will not reimburse these visits.
If you have Medicare, we’ll guide you on which labs and services can still be covered elsewhere.
Labs & Testing

Yes. We try to route lab orders through your insurance when possible. However, every insurance plan is different, and we cannot guarantee what your insurer will cover or how much they will reimburse. We recommend checking directly with your insurance company for the most accurate coverage and cost information.

Yes. If you prefer upfront, discounted pricing, we also partner with a direct-to-consumer lab that offers clear self-pay rates and multiple convenient blood-draw locations. Many patients choose this option for cost transparency and speed.
We’ll review both options with you at your visit so you can choose what works best for your care and your budget.
Hormone & Midlife Care

Yes. We specialize in bioidentical hormone replacement therapy (HRT)— estrogen, progesterone, and testosterone — when clinically appropriate.
We use FDA-approved or carefully compounded formulations for precise dosing and easy adjustments. We do not place hormone pellets. We can provide care to women who want to transition off hormone pellets.

Perimenopause, menopause, thyroid concerns, weight management, sexual health, vulvar conditions and overall midlife wellness.

Yes. Men are welcome for testosterone replacement therapy (TRT) and medical weight loss with our Family Nurse Practitioner, Holly DeWalt, FNP.
Holly specializes in men’s and women’s hormone optimization and weight management.Please note that Jean Marino, WHNP-BC, and Ann Konkoly, WHNP-BC, do not see male patients—but Holly is happy to see your husband or partner and provide the same evidence-based, personalized care we offer all patients.
Appointments

Initial visits are 60 minutes; follow-ups are typically 30 minutes. We focus on education, shared decision-making, and giving you plenty of time to feel heard and supported.

Yes. We have membership options for ongoing support, priority scheduling, and special pricing on services.

Yes. Many appointments, including hormone consultations and follow-ups, can be done virtually. Procedures and some initial visits are in-office.

Because we reserve significant time for each patient, we ask for 48 hours’ notice to cancel or reschedule. Late cancellations or no-shows may incur a fee. Please text the office at 216-877-8879 for cancellations.
